Completed Visa application form with one recent passport
size photograph (Less than six months old.)

A
passport valid at least one year before the date of
expiration. Passport must contain sufficient blank visa
pages.

Copy of travel itinerary or round trip ticket should be
produced. In some instances, the Consular Office will
invite applicants for interview.

For Applicants that are minors, both parents must sign
the Form, the child’s birth certificate attached and a
Notary Public must notarize the Form.

If
applicant is mailing the passport, a prepaid
self-addressed envelope should be enclosed.
ONLY
U.S. POSTAL EXPRESS MAIL IS ACCEPTED.

A
valid and up-to-date International Certificate of
Vaccination (Yellow Fever).

Business Visa Applicants:
A
profile of the company the applicant represents with a
letter of invitation from a registered Company/Organization
in Sierra Leone is required.

Evidence of sufficient funds to cover the period
applicant wishes to stay in Sierra Leone (e.g. copy of
bank statement)

There is no limit to the amount of money carried into
the country but the amount should be declared at the
port of entry.

All fees must be made payable to the Embassy of the
Republic of Sierra Leone either in Money Order or
Cashier’s Checks. NO PERSONAL CHECKS are accepted.

The
Embassy authenticates documents such as Birth Certificates,
Death Certificates, Passports, Educational and business
documents, driver’s license etc., that have been issued by
the various authorities in Sierra Leone.

3. Republic of Sierra Leone Passport Application
Forms $10.00
The
Embassy no longer process application for the issuance or
renewal of ordinary passports. However, Application Forms
for the issuance of these passports can be purchased for
$10.00 at the Embassy.
Applicants for replacement of passports should forward
copies of the first five pages of the expired passport,
money order of $10.00 and a prepaid
self-addressed envelope. Applicants that have never had a
Sierra Leone passport must present their original Republic
of Sierra Leone Birth Certificate.
4. Lost/Stolen Republic of Sierra Leone Passport:
